REQUEST CARE
The Indication and Allocation Committee for Residential Care for People with Intellectual and/or Multiple Disabilities is the body on the island that is responsible for the indication, allocation and placement of new clients within the local care for the disabled.
The process consists of filling out an application form and conducting an intake. The completed application form together with a copy of the ID and the AZV pass will be sent to the committee for further processing.
THE INTAKE
If the application meets all the criteria, an intake interview will follow at your home by a representative of the indication committee. During the intake, an inventory will be made of the current living situation and care needs. Based on this, an indication and allocation can take place, in which the degree of urgency can also be determined.
INDICATION AND ASSIGNMENT
You will be informed in writing of the decision of the indication committee.
In the decree you can read about both the indication and allocation. If a care place is available at the assigned care provider, it is also possible to make short-term agreements with the care provider regarding the placement.
CURATORSHIP
Our clients over the age of 18 with intellectual disabilities are unable to make personal decisions or manage their own financial affairs. To prevent abuse of this, a trustee can be appointed. This person makes both financial decisions for the person concerned and decisions about care. Ambiente Feliz cannot mediate in this, but wholeheartedly recommends it.
The foundation asks for an annual contribution for activities of 250 AWG.
